DO YOU ACCEPT INSURANCE?

 

Milestones On The Go is private pay and an out-of-network provider for all insurance carriers.

 

Finding a therapist that is the "just right fit" for your child is "priceless!"

We are keenly aware, however, that therapy can get expensive.

We desire to keep our fees competitive while providing treatment that is not dictated by insurance carriers. 

At Milestones On The Go, it is our desire to partner with you and your child and it is our promise to always provide ethical, evidence based treatment and appropriate discharge planning.

HOW CAN I PAY FOR OCCUPATIONAL THERAPY SERVICES?

We will gladly prepare a superbill free of charge upon your request, should you desire to submit to your out-of-network insurance benefits for reimbursement. Please be aware that not all employers offer out-of-network benefits.

A superbill is a detailed receipt of services provided as well as information insurance companies need to create a healthcare claim. You will submit the superbill to your insurance company directly for any reimbursement due to you. Please note, Milestones On The Go does not negotiate claims with insurance companies. This will be your sole responsibility. 

If your child is homeschooled, they may be eligible for a waiver to pay for therapy services.

 Milestones On The Go, is committed to providing best practice, evidence-based treatment while keeping costs competitive. 

*We require full payment for your evaluation and treatment at the time of service, and you will be reimbursed directly by your insurance carrier any amount that is due to you after your claim has been processed. 

** Therapeutic Listening and Interactive Metronome can be provided as a treatment modality during therapy sessions as indicated OR as individualized full protocol programs.  

Additional fees apply for full protocol delivery of these services.

  • 18 years of age or younger

  • No existing insurance coverage for requested expenses

  • Household income per child may not exceed: 

    • 1 - $40,000​

    • 2 - $55,000

    • 3 - $70,000

    • 4 - $85,000

    • 5 - $100,000

United Healthcare Children's Foundation

Eligibility criteria:

  • 16 years of age or younger

  • Household income per family member:

    • 2 - $55,000

    • 3 - $85,000

    • 4 - $115,000

    • 5 or more – $145,000

  • Commercial health insurance

C.A.R.E.

Eligibility criteria:

  • Autism diagnosis

  • Household income level less than $75,000 per year
